Vanilla one (BTR-K Kamysh) works fine with exactly the same waypoints and squad (had to delete two guys who were over-capacity though).
The BMP-1 doesn't seem great at following waypoints in general (for instance, in the attached mission the "Crewed BMP-1 + Separate squad" one will tend to overshoot its "Transport unload" waypoint by a hundred meters or so) so maybe it's somehow a PhysX issue? 🤔

+ UAZ searchlight operation
thats physx being a cunt, unlikely it will be resolved.
Can we just say, pull BMP-1 physx on the MT-LB and call it a day?
On the side note, MT-LB should not have the built-in GPS.
There's a relatively major PhysX issue - the MTLB accelerates when going uphill, without any input from player, as if it's going downhill.
I can release the [W] button and the thing will still accelerate until it reaches its max speed.

Make turret-less ambulance version
This can also be a civilian version - those are freely available in Russia and sometimes used by hunters or those living in northern Russia. They usually come in Russian Green paint (already present in CUP).
Unrelated to PhysX — the turret MG can be raised way too high, to almost 85 degrees, clipping through the turret's roof.
PhysX configuration could really use some refactoring.

I'm afraid I don't quite follow 😕 From the looks of it the muzzle flash appears at the right place. It isn't supposed to appear at the muzzle of a main cannon as the rounds aren't fired through it but through the AGS-17 launcher mounted to the top of a turret.
The position of the flash is correct. The weird thingy at the end of which a flash appears is the AGS-17. The design of the launcher was a bit reinterpreted by BI but that's exactly where grenades are supposed to be fired from, not through the main cannon.
